How to Register with the Practice

We can only accept patients who reside in our practice area shown below. Please advise the Receptionist that you wish to register and she will arrange appointments with the Doctor for your registration, and with the Healthcare Assistant for your New Patient Check. Please do not be put off by this. It is a simple procedure in which you will have your blood pressure checked, your urine sample tested and a few questions asked about your personal and family medical history.

You will need to download a New Patient Health Questionnaire, which needs to be filled in and brought to the Surgery, along with photographic ID and proof of address, where you will be asked to fill in another short form and booked an appointment to see the Healthcare Assistant and Doctor. If you have a medical card, please check that the details are correct, sign it and bring it along with you. If you have lost your medical card, you will need to complete forms prior to seeing the Doctor. Please allow plenty of time to complete the paperwork before your appointment time. Please also bring a specimen of urine with you at your first appointment.

The Doctors work as a partnership in the Practice, which means that you are able to see any of the partners or a specific one, at your preference.

The Practice covers the NP19 area of Newport, and also the following areas: Caerleon, Llanwern Village, Nash, and Goldcliff.

We can only accept and care for patients who reside within the boundaries of our Practice areas.
