Information for Carers

A carer is someone who provides informal care to a relative or friend who because of disability, sickness or frailty is unable to live at home without support.

At Park Surgery, we understand the pressure that can be put on carers and like to think of ourselves as a "Carer Friendly" doctors surgery. If you are a patient of Park Surgery and have a carer, or are a patient who cares for someone else it is beneficial for us to know about this. You can either pop into the surgery and fill in a form to tell us, email us via the contact page, or download and complete our Carers Identification Form and send/email it back to us.

Carers Contact is an information resource in Newport Market, High Street, funded by Newport City Council for the benefit of the carer and the cared for. Please feel free to download a copy of their Carers Contact Leaflet or Carers Information Booklet. You can also find out about the Emergency ID Card for Carers, provided by Newport City Council.
